When his time was up, he continued the fast back at home, returning to the hospital only for regular monitoring. After a week, he was down five pounds and feeling good. His vitals checked out, blood pressure was normal, and though he had lower blood sugar than most men, he didn’t seem particularly impaired by it. The experiment continued. All told, he lost 2. Over the five following years of observation, AB regained just sixteen pounds, putting him in excellent, but underpopulated territory (at least 8. Other doctors paid attention. Maybe it was the fact that it was the 6. Vietnam, Ken Kesey and his Merry Pranksters blazing across the U. S. Study after study shows that whatever you want to call the protocol – intermittent fasting, fasting, alternate day fasting, or alternate day caloric restriction – it works very well for weight loss. A few recent ones: So, yes: it works. But does fasting work solely through caloric restriction, or is it doing something special? That’s the real question. There’s no question that fasting causes weight loss through caloric restriction. Obviously, when you don’t eat anything, your body turns to its own stored energy reserves, reserves that take up physical space and have mass. Depletion of those energy stores reduces mass and thus weight. Total and absolute caloric restriction. That’s elementary stuff and the studies from the 1. To dig a bit deeper, let’s look at how weight loss occurs during a fast. I’ll stick to research involving humans only (sorry, rodent personal trainers). Secretion of growth hormone, one of the premier fat burning hormones, increases during a fast. In a five- day fasting protocol, men experienced increased GH secretion on day one and day five (the only two days where GH was measured). A later study showed that during two- day fasting sessions, growth hormone secretions increased in both frequency and intensity in men. They experienced more frequent GH bursts and each burst secreted a higher mass of GH. A more recent study found that 2. GH by 1. 30. 0% in women and almost 2. Fasting decreases fasting insulin levels. The presence of insulin inhibits lipolysis, the release of stored triglycerides (body fat). Without lipolysis actually releasing stored body fat, it’s rather difficult to, well, burn that body fat for energy. During a fast, fasting insulin decreases and lipolysis increases. This insulin- blunting aspect of fasting quite literally allows the fast to be successful, because without the ability to access stored body fat for energy, making it through a period of zero caloric intake will be nigh impossible. Fasting improves insulin sensitivity. Fasting increases the catecholamines, both adrenaline (epinephrine) and noradrenaline (norepinephrine). Both catecholamines increase resting energy expenditure during a fast, and guess where your fasting body finds the energy to expend? From body fat. Catecholamines activate hormone sensitive lipase present in adipose tissue, spurring the release of said fat. This makes intuitive sense, doesn’t it? If you’re hungry in the wild, you need to hunt (or gather, or fish, or somehow procure food) and you need energy to do it. The catecholamines help provide some of that energy while burning fat in the process. Hmm, notice anything? All those mechanisms dealt with fat burning specifically. While there may be some weirdo out there who’s interested in reducing bone mineral density and muscle mass while maintaining fat tissue, I would wager that what most people mean by “weight loss” is “fat mass loss.” From the stuff I just linked, it looks like fasting burns fat, rather than just weight. But what about Conventional Wisdom which claims that fasting increases muscle wasting – maybe because your body will totally recognize the lethal nature of all that arterycloggingsaturated animal fat and choose to break down muscle instead? Is it true? Let’s go to the research: In one study, normal weight subjects ate just once a day without reducing overall caloric intake. Weight didn’t change, which isn’t really surprising, but body composition did change – and for the better. Body fat decreased and lean weight increased (in addition to a bunch of other beneficial changes) without an overall reduction in calories. A recent review of the relevant literature found that while fasting and caloric restriction are “equally as effective in decreasing body weight and fat mass,” fasting is “more effective for the retention of lean mass.”Conventional Wisdom strikes out again. In closing. It decreases caloric intake. In order to lose weight, you need a caloric deficit. That really isn’t in contention here, folks. It increases fat oxidation while sparing lean mass. Since what we’re trying to do is lose fat (rather than just “weight”), the fact that fasting increases hormones that preferentially burn fat and decreases hormones that inhibit fat burning is extremely desirable. It improves adherence. In most of the studies surveyed, participants found fasting to be an extremely tolerable way to diet, especially when compared to outright caloric restriction. Even AB, the fasting Scotsman, reported very little difficulty throughout his 3. If fasting is easier for you than trying to laboriously count calories, fasting is going to be the more effective weight – er, fat – loss method. All in all, fasting is an effective way to lose body fat. It’s not the only way, and it isn’t “required” for Primal weight loss, but many in the community have found it to be very helpful and the literature backs them up. Opinion: Alternate- day fasting vs. It’s a new diet strategy where you eat restrictively on some days but then eat freely, almost indulgently, on others. Although many forms of intermittent fasting exist, the most common form I’ve seen is what’s called alternate day fasting. With alternate day fasting, you can eat very little on one day (usually one 5. The idea has an obvious appeal. Dieting is hard because eventually, people find the restrictive nature of most diets impossible to stick with and revert to their old ways. But alternate day fasting seems easier because most people can summon enough will power for one day if they know they can be libertines tomorrow. The obvious question becomes, does intermittent fasting work? Will it help you lose weight? And perhaps most important, does it outperform a standard diet where you just eat a little bit less every day? That question was answered recently in a study published in JAMA Internal Medicine. Researchers took 1. The first group was told to continue their regular habits. The second intermittent fasting group ate a single 5. The third group was told to follow a conventional diet where they simply ate less every day. The results were a surprise to the researchers. There was no difference between intermittent fasting and conventional diets. Both groups did equally well. Or if you are a nihilist, both groups did equally badly. At six months, both groups had lost just under seven pounds. By one year, the number had slipped back to about six pounds. These numbers probably don’t seem that impressive. On TV, you can drop 3. In real life and clinical trials, 5- 1. But intermittent fasting proved to be a disappointment in this trial. It did not outperform its conventional counterpart. In terms of weight loss, visceral fat, blood pressure, blood sugar, insulin levels and inflammatory markers, there was no difference between intermittent fast and simply eating a little less on a daily basis. In fact the intermittent fasting group had slightly higher cholesterol levels at one year, although the difference was minimal. Here, intermittent fasting underperformed because rather than being easier to stick with, it ended up being harder. More people abandoned the intermittent fast (3. In the end, intermittent fasting is not better than a standard diet. It won’t help you lose more weight and it won’t make you any healthier. Though to be fair, it’s not much worse, either. And contrary to the hype, it actually seems to be harder to stick with long term, not easier. I’m sure some people will swear by intermittent fasting and say that it works for them. The point here though is not that one diet is better than other. It’s that diets work only if they get you to eat less. Eating less is really the only way you can lose weight (although regular exercise certainly won’t hurt). How you do it isn’t really as important as whether you do it. Fad diets fail because they are unsustainable and people abandon them. So when I’m asked what’s the best type of diet, the answer is clear. The best diet is the one you can stick with. Researchers have found that people who stuck to a diet that included foods like berries, leafy greens, and fish had a major drop in their risk for the memory- sapping disorder, which affects more than 5 million Americans over age 6. The eating plan is called the MIND diet. Here’s how it works. Brain- Friendly Foods. MIND stands for Mediterranean- DASH Intervention for Neurodegenerative Delay. It’s similar to two other healthy meal plans: the DASH diet and the Mediterranean diet. But the MIND approach “specifically includes foods and nutrients that medical literature and data show to be good for the brain, such as berries,” says Martha Clare Morris, Sc. D, director of nutrition and nutritional epidemiology at Rush University Medical Center. You eat things from these 1. Green leafy vegetables (like spinach and salad greens): At least six servings a week. Other vegetables: At least one a day. Nuts: Five servings a week. Berries: Two or more servings a week. Beans: At least three servings a week. Whole grains: Three or more servings a day. Fish: Once a week. Poultry (like chicken or turkey): Two times a week. Olive oil: Use it as your main cooking oil. One study showed that people who stuck to the MIND diet lowered their risk of Alzheimer’s disease by 5. On the other hand, people who followed the DASH and Mediterranean diets “moderately” had almost no drop in their Alzheimer’s risk, Morris says. Scientists need to do more research on the MIND approach, “but it’s a very promising start. It shows that what you eat can make an impact on whether you develop late- onset Alzheimer’s,” which is the most common form of the disease, says Cecilia Rokusek, a registered dietitian at Nova Southeastern University. Continued. Should You Follow the MIND Diet? Even if you don’t have a family history of Alzheimer’s disease or other risk factors, you may still want to try this eating plan. It focuses on nutritious whole foods, so “it’s not just good for your brain. It’s good your heart and overall health, too,” says Majid Fotuhi, MD, Ph. D. He is the chairman and CEO of the Memosyn Neurology Institute. One of the best things about the plan is that you don’t have to stick to it perfectly to see benefits, Rokusek says. Moderator: Welcome to Web. MD Live, Dr. Is memory loss an inevitable part of aging? People may have slower rate of learning and memorizing things, but they should not lose their memory. Some degree of forgetfulness is normal with aging, but people should maintain the ability to function in their jobs and remember names of their spouses, children, friends, and so on. The only thing they should not forget is the names of their close relatives and their friends. That would be abnormal. Moderator: How can one determine what is causing short- term memory loss? Fotuhi: The most common cause of memory loss is stress and anxiety. The second most common cause is depression. The third most common cause is medical issues. Only the 1. 0th or 1. Alzheimer's disease. Ninety percent of older adults who complain about memory loss do not have Alzheimer's disease. Most of them have depression, stress, anxiety, fatigue, and lack of sufficient amount of sleep or medical issues. Member question: What, if any, is the difference between dementia and Alzheimer's disease? Fotuhi: That is a very good question. Dementia means memory loss plus deficit in one or more area of cognition, such as getting lost, confusion of time, or inability to do the usual hobbies a person may have. There are many causes for dementia, such as high blood pressure causing vascular dementia. The most common cause of dementia is Alzheimer's disease. So Alzheimer's is one of several different types of dementia. Moderator: So what would be the first step a person should take if they feel they are losing their memory? Fotuhi: The first thing they should do is to check the things I mentioned, such as stress, fatigue, lack of sleep, and most importantly, depression. They should discuss their memory loss with their physician. Most doctors can determine whether a person's memory loss is due to medical issues or if it is due to dementia. So check your causes of memory loss and talk with your doctor. Member question: Dr. Fotuhi, my mother suffered a stroke at age 6. Having had a stroke myself at age 5. I at a higher risk to develop vascular dementia? Fotuhi: Yes. People who have vascular risk factors are at a higher risk or developing dementia. Having high blood pressure, high cholesterol, and high homocysteine all contribute to developing heart attacks, a stroke, and vascular dementia. Fortunately, all these risk factors are treatable and they should be treated aggressively. Once they are treated, a person is no longer at risk for developing dementia, especially if they start in their midlife. Member question: I am a 5. How can I tell if the memory problems that I have been experiencing over the last year or so are some form of Alzheimer's or not? My grown children talk about events from their childhood that I just don't recognize. I also forget things as soon as I hear them and I lose words - -I know what I want to say, but cannot remember the words. At times, familiar places become foreign to me and I can get lost or disoriented walking or driving. Fotuhi: That is actually worrisome for dementia. However, the cause of dementia may be depression, which can be treated with medications, so I strongly recommend that you discuss this issue with your doctor. Member question: I am 4. I remember faces of students taught but not their names. Sometimes I scan my memory to remember something that has just happened but everything is blank. I call someone to pass on a message; by the time the person turns around (a matter of seconds) I have already forgotten the message to be passed. I had a surgical hysterectomy three years ago. Could it be the cause of memory loss? I tend to preserve the long- term memory but the short term one - - it's almost chaotic. Fotuhi: Hysterectomy does not cause memory loss. The kinds of memory lapses you describe are very common. In other words, you may lose memory temporarily but you can retrieve that memory later. The kind of memory loss that would be concerning would be the type that the person would completely forget all the information. For example, it would be OK to forget the name of a movie you saw last weekend. However, it will not be OK for you to forget that you saw a movie at all Again, there are many causes for short- term memory loss; the most common causes being stressful lifestyle, lack of sleep, anxiety, and depression. The chance that you have Alzheimer's disease in your 4. My advice to you is ? Fotuhi: Alzheimer's disease is not as common as people think it is. Only one in 1. 00 of those in their 6. Among those in their 7. Alzheimer's. The numbers are much higher only for those in their 8. Member question: I have memory loss due to an accident where I fell and hit my head. Can it be reversible? Fotuhi: Head trauma associated with coma is one of the many risk factors for memory loss that comes with aging. However, other risk factors, such as high blood pressure, high cholesterol, high homocysteine, lack of exercise, lack of education, and depression, are more likely to contribute to memory loss than a single head trauma. If you did have a serious head trauma, you should focus on reversing your other risk factors, since you cannot reverse having had a head trauma. You can also make sure that you prevent the chance of having a future head trauma for yourself and especially for your children by making sure that they wear their seat belts and wear protection for their head when they are involved in contact sports. It is very important to wear a helmet when you or your children are skateboarding, biking or especially of you're on a motorcycle. Member question: Can menopause cause memory loss? Fotuhi: There are. I think if a woman. For example, low thyroid levels are common and. Taking hormone replacement therapy is now shown not to be. So the short answer is, check your reversible risk factors and try to maximize your memory ability by exercise and better diet. Moderator: What is the role of exercise and diet in memory loss? Fotuhi: Exercise improves memory in two ways: It improves your heart function. This means that your heart can pump the blood better and profuse your brain with a rich supply of blood. Growth factors are proteins that naturally nourish the brain cells and help them repair small injury and remain in good health. By increasing the levels of these growth factors, exercise makes your brain a better working machine. Member question: What are these growth factors? Fotuhi: One of them is called BDNF, which stands for brain drive neurotrophic factor. Another growth factor is called nerve growth factor or NGF. Both these growth factors have been shown to increase with exercise and to improve memory. Member question: I am 5. I was treated for clinical and postpartum depression in my 2. ECT - - quite a bit of ECT - - over a period of five years, but not constant for those five years. Umpteen professionals told me that I would only have short- term memory loss and that it would return. Well, they either lied, mislead me, or did not know what they were relaying to me because ECT erased 9. Can you give me an honest answer as to why I lost my memories? Also, my memories of me growing up were erased. I live in the present and it is very upsetting to me that I cannot discuss my kids growing up with them. To say the least, I am angry and very sad. Fotuhi: I appreciate how frustrating it must be to have a very poor memory. To the best of my knowledge, there is no formal study that has shown ECT to be associated with memory loss. I recommend that you discuss with your physician the other reversible causes for memory loss I mentioned earlier. You may also need to have a brain MRI. Depression, as I mentioned earlier, is one of the main causes of memory loss. So the difficulties you have with your memory may still be due to depression. I think that you do need a full medical workup to identify the cause of your memory loss. Member question: What vegetables/herbs do you recommend for helping progressive memory loss. Items that I can grow myself here in southern Texas. Assuming I can remember where I last left the shovel and hoe. Fotuhi: I have a list of foods that are rich in antioxidants and vitamin in my book, The Memory Cure. I can give you a short list, and those include: Spinach. And especially blueberries or other berries I also recommend two or three servings of fish a week. Several studies have shown that people who eat fish two or three times a week have a lower risk of developing Alzheimer's disease. You also need plenty of B1. Again, a complete list of these is on page 1. Moderator: Let's talk about preventing memory loss. You mentioned foods. How about education and activity? Member question: Do you agree with the ? Fotuhi: The answer is absolutely yes. Use it or lose it really applies to your brain as much as it applies to your muscles. Several studies in the past two or three years have confirmed this notion. People who do more crossword puzzles, play chess, or enjoy activities that require thinking, are less likely to develop Alzheimer's disease. I often find it surprising on how well my patients do if they had a strong history of education and if they maintain an interest for learning. Even when they do develop Alzheimer's disease in their brain, they continue to function fairly well. Sometimes when we look at the brains of some of our participants in the Alzheimer's disease research center after they die, we find that they had Alzheimer's disease in their brain, but did not have any signs of dementia when we last saw them. |
